Our Office

Nestled around the sleepy San Lorenzo River and guarded by towering redwoods, Ben Lomond offers its own unique culture. It is home to Highlands Park which formerly hosted the annual Highland Games Festival. In Park Hall the Mountain Community Theater puts on plays several times a year. Local color is on display at establishments like Ristorante Casa Nostra and the Santa Cruz Mountains Art Center.

Big Basin State Park to the north and Roaring Camp to the south, as well as the many hiking trails and swimming holes, provide easy access to enjoying this most beautiful region.

We are proud to offer our neighbors in the San Lorenzo Valley and surrounding areas over 20 years of experience in dental care.

New and existing patients are very warmly welcomed by our caring staff and treated with a gentle approach to all dentistry to allow our patients a positive experience.

To make it easier for new patients to get started, we have made all the patient forms available online which can be downloaded and filled in before visiting our practice. When you reach our practice, one of our team members will assist you with all the requirements such as form submission, Insurance claim cashless treatment, etc. Your appointment with the dentist can take 30- 90 minutes depending upon the type of treatment.

Insurance and Payment Options

We currently offer the following payment options for treatment performed in our office:

As a courtesy to our patients, we will take care of sending claims to your insurance company for you. We accept most PPO insurance plans even though we are ONLY contracted with Delta Dental Premier. We are not contracted with Medicare, Medi-Cal or any HMO plans, including DeltaCare USA.

As a contracted Premier provider with Delta Dental we have agreed to charge fees based strictly on our contracted Premier fee schedule. Patient portions are based on percentage of insurance coverage, less an annual deductible (if applicable). Most insurance companies have individual plan limitations, frequency limitations and plan annual maximums. New plans may also have “waiting periods” for certain services. We are happy to call your insurance company and find out the specifics for your plan. It’s also a good idea to read through your insurance packet to familiarize yourself with the limitations and benefits of your plan. For all non-Delta plans we are considered Out Of Network. To limit surprise costs please check with your insurance company to see if they pay Out of Network providers at the UCR (Usual Customary Rate, AKA our billing rate) or a Max Allowable Fee Schedule.

As a courtesy to you, we will bill your insurance company but you are responsible for the fees charged by our office, no matter what your insurance coverage may be. We do our best to provide accurate estimates but are not at fault for denial of coverage. Please keep us informed of any changes to your insurance plan.

While we make every reasonable effort to give our patients accurate cost estimates before treatment, we remind you that insurance policies are agreements between patients and insurance companies (not doctors and insurance companies).

Pre-treatment estimates can be sent to your insurance company before treatment is done. This is only done on request, so notify us if you would like to be sent a pre-treatment estimate. Also note that some insurance companies can take 30-60 days to process these requests. So please be patient with us.

Cash Patients (Those Without Insurance)
We have many patients who do not have dental insurance coverage. We are able to offer a 5% discount to cash patients for paying the complete balance the same day as treatment.

Care Credit
We accept Care Credit, which is a credit card provided for health care services. Care Credit offers NO Interest plans, which can be spread over 6, 12, 18 or 24 months depending on the amount charged, with monthly payments as low as $25.00. Care Credit is accepted in many healthcare provider offices. It is easy to securely apply online and can even be done from your smartphone.

Dental Discount Plans
Dr. Creech is not currently contracted with any dental insurance discount plans. We do however offer an in-house discount plan. Please see https://www.menu.dental/creechdental for more information.

Dr. Creech is proud to serve the needs of our Veterans. Although she is NOT currently contracted to work with The Department of Veterans Affairs.

Cancellation/Missed Appointment Policy

We value our patients’ time and have reserved time especially for you in our schedule. Should you need to change or cancel an appointment, we require 48-hour notice. This advance notice allows us the time needed to call other patients wanting to be seen and offer them your appointment slot.

Our late cancellation fee is $75 per hour scheduled.
